Are Health Care Workers Who Decline Flu Shots Irresponsible?

More than a third of U.S. health care employees were not vaccinated last flu season. Research shows that the unvaccinated staff have a decent chance of getting sick--and passing that infection on to at-risk patients

MALTA?U.S. drug stores have been advertising their seasonal flu shots to the public for weeks, and here at Scientific American employees have already been invited to sign up for free jabs next month.

These vaccination enticements, for the most part, come down to individuals' decisions about arming themselves against the flu. But for doctors, nurses and other workers in the health care industry, their level of protection against the season's circulating flu strains has the potential to impact hundreds of others, noted health experts at the fourth European Scientific Working Group on Influenza (ESWI) conference earlier this month in Malta.

If an office worker gets sick, he or she might spread germs to co-workers who sit nearby. But if doctors or nurses get the flu, they can pass it to the dozens of patients they come into contact with each day?many of whom have other conditions that render them more susceptible to infection. The flu still kills thousands of people each year in the U.S., most frequently those with underlying conditions, who also have more frequent contact with medical personnel than the healthy.

Last winter just under two thirds (63.5 percent) of health care workers got a seasonal flu shot, according to an August report published by the U.S. Centers for Disease Control and Prevention (CDC), even though universal vaccination of health care workers is supported by numerous scientific and medical groups.

Why would so many of these workers forgo the protective injection? "There is some question of understanding" as to whether even many health professionals are aware of the limited risks and outsize benefit of the vaccine, Maria Zambon, director of the Center for Infections at the U.K.'s Health Protection Agency, said at ESWI.

Many of those in the health care field who opted not to get the shot did not think that the vaccine "was worth the time and expense" or that it could "protect them and the persons around them from the disease," according to the CDC report. About 40 percent of the health workers who did not get vaccinated said that they did not consider influenza a serious threat to the health of people around them?and about 55 percent said that they did not think the vaccine would protect those around them.

And as tough as they can be, the immune systems of doctors and nurses are not impervious to influenza. Unvaccinated health care staffers are second only to unvaccinated adults living with kids in their likelihood of coming down with the flu. About 18.7 percent of unvaccinated health workers versus 24 percent of unvaccinated adults with children in the home get the flu each year, according to Allison McGeer, a microbiologist and infectious disease consultant at Mount Sinai Hospital in Toronto. That means "health care workers are at higher risk of influenza than other working adults," who have an annual infection rate of just 5.4 percent, she said at the ESWI meeting. And her analysis suggests that health workers tend to have a higher rate of asymptomatic infections, which "may say something about the risk of influenza transmission to patients."

Hospitals mandating vaccination for employees have raised the hackles of some unions and professional associations. States and local governments can also exercise their rights to pass mandatory vaccination laws for certain groups (such as school children, military enlistees or health care workers), according to a congressional report (pdf) produced earlier this year.

For medical institutions that have adopted a requirement that their employees get the shot, their rate of vaccination was 98.1 percent last flu season, according to the CDC report. Another study found hospitals that introduced vaccination requirements mid-season last year increased the rate of employee flu shots 62 percent to 76.6 percent. "Hospitals that are unable to improve suboptimal influenza vaccination coverage through multifaceted, voluntary vaccination campaigns may consider institutional requirements," noted the authors of the paper, which was published online September 22 in Vaccine. "Rapid and measurable increases in vaccination coverage followed institutional requirements." These employers still seemed to be in the minority, though, with only 13 percent of respondents in the CDC study reporting that they had been required to get the shot.

Another policy that has boosted shot rates in some hospitals, just shy of a full-coverage mandate, is the condition that any employees who decline the vaccination be required to wear a surgical mask for the duration of the flu season?even if that employee does not work with patients. "That means that individual is stigmatized, whether or not the face masks are doing a good job of preventing transmission," Arnold Monto, a professor of epidemiology at the University of Michigan School of Public Health, said at the ESWI. The mask policy, for example, increased uptake of the vaccine from 70 to 96 percent at one Massachusetts hospital, The Boston Globe reported earlier this month.

Improving uptake of the seasonal flu vaccine among health workers could also improve response to a pandemic in the future by creating wider immunity to a number of strains and by habituating people to getting the shot. "If we get it right for influenza, it will help us to be better prepared for other infections," Zambon said. But, she added, those in the health care industry are a crucial barometer: "If we are unable to convince them about the vaccine," the rest of the population might be even more challenging to rally. During the 2009 H1N1 pandemic, for example, only slightly more than a third (37 percent) of health care professionals ended up getting the shot when it became available.

Monto calls the decision to pass up the vaccine?whether you are a health worker or not?"stupid behavior," noting that the demonstrated benefits to yourself and those around you far outweigh any slight risks of adverse reactions.

McGeer put it a little more diplomatically, noting that there is "no question in my mind, from a scientific point of view, that no one should be working in health care without being vaccinated."

Libya finds mass grave from 1996 massacre (Reuters)

TRIPOLI (Reuters) ? Libya's interim rulers said on Sunday they had found a mass grave containing the bodies of 1,270 inmates killed by Muammar Gaddafi's security forces in a 1996 massacre at a prison in southern Tripoli.

To the east of Tripoli, NATO bombers hit the city of Sirte to clear the way for fighters with the National Transitional Council (NTC) who are trying to capture Gaddafi's hometown.

But Gaddafi loyalists showed they were still a threat by attacking the desert oasis town of Ghadames, on the border with Algeria, NTC officials said.

The mass grave was the first physical evidence revealed so far of the Abu Salim prison massacre, an event that was covered up for years but created simmering anger that ultimately helped bring about Gaddafi's downfall.

"I am happy this revolution succeeded, and that our country will be better," said 45-year old Sami al-Saadi, who believed two of his brothers died in the massacre. "But when I stand here, I remember my brothers who were killed."

According to accounts from survivors who have spoken to human rights groups, starting at dawn on June 29, 1996, guards lined up inmates in the courtyards of the Abu Salim prison.

Security men, standing on the prison rooftops, fired at the inmates with Kalashnikov rifles before using pistols at close range to finish them off.

The uprising that toppled Gaddafi was ignited by protests linked to the Abu Salim massacre. In February, families of inmates killed there demonstrated in the eastern city of Benghazi to demand the release of their lawyer.

NATO JETS

Anti-Gaddafi forces had pushed to within a few hundred metres of the center of Sirte, one of the last bastions of pro-Gaddafi resistance in Libya, but later drew back to let the NATO jets do their work.

"Yesterday our freedom fighters attacked Sirte city from two sides. That doesn't mean that Sirte is free now, but it is an indication that Sirte will be free soon," said Ahmed Bani, NTC military spokesman in Tripoli.

"I'm asking now any militiamen fighting on the side of the tyrant (to realize) that the game is over."

On Sunday, the roar of jet engines could be heard overhead, as well as sporadic booms when NATO ordnance hit targets on the ground. One strike, giving off a deep thud, released a big cloud of smoke and dust over the south of the city.

There was little fighting on the ground west of Sirte, where NTC fighters have advanced closest to the city center.

On the eastern side, the forces pushed to within 15 km (9 miles), an advance of more than 25 km. A Reuters reporter there said the NTC forces had been helped by heavy NATO bombing.

She said she could hear the sound of artillery fire and see black smoke on the horizon. Doctors at a hospital east of Sirte said one fighter had been killed and 12 wounded in clashes on Sunday.

Taking Sirte would be a huge boost for the NTC as it tries to establish credibility as a government able to unite Libya's fractious tribes and regions, and a blow for Gaddafi, widely believed to be on the run inside Libya.

Offering what may be a glimpse into the deposed leader's state of mind, his spokesman contacted Reuters to deny reports that Gaddafi and his family had helped themselves to Libya's oil wealth.

"The leader of the revolution and his family are among the poorest citizens," said the spokesman, Moussa Ibrahim. He spoke by telephone and did not reveal from where he was calling.

Accounts from NTC fighters and people who had left Sirte indicated pro-Gaddafi forces were trying to prevent civilians from fleeing, effectively using them as human shields.

"Gaddafi's forces have surrounded the area, closed it off, by shooting at people," said a man called Youssef, who was driving away from Sirte with his wife. "There are a lot of people who want to get out but can't."

A man who said he was a doctor at a hospital in the center of Sirte said by telephone it was the NTC forces who were causing civilians to suffer.

He said wounded people were dying daily because medical supplies were running out and part of the hospital had been hit by shellfire.

The doctor, who gave his name as Abdullah Hmaid, spoke to Reuters using the mobile telephone of the Gaddafi spokesman, Ibrahim, who is a native of Sirte.

"The armed opposition refuses to cooperate with us and is imposing a health blockade on the city," said Hmaid.

"We call on the Word Health Organization, the International Red Cross and Amnesty International and human rights organisations to work to break the siege of more than 200,000 people in Sirte."

FRAGILE GRIP

The attack by pro-Gaddafi forces on Ghadames underlined the fragility of the NTC's grip even on parts of the country nominally under its control.

The town, about 600 km south-west of Tripoli, is near a border crossing that pro-Gaddafi Libyans have used to flee into Algeria. Its old town, an intricate maze of mud walls, is listed as a UNESCO world heritage site.

"These militias have attacked our people in Ghadames city ... All the information we have got is that these groups are related to the son of Gaddafi, Khamis," the NTC's Bani told a news conference.

"Our freedom fighters have taken control of that area," he said, though he acknowledged the clashes were not completely over. "This problem will end soon. It's a matter of days."

A month after ousting Gaddafi's forces from Tripoli and most of the country, the NTC is now facing challenges to its rule from only two main locations, Sirte and Bani Walid, a town about 170 km (105 miles) south-east of Tripoli.

Until both are captured, Libya's new rulers say they cannot begin the process of holding the first elections.

That leaves the country in limbo where the only real authority comes from disparate factions of anti-Gaddafi fighters who are still armed and want a stake in the new Libya.

Deepening the uncertainty, the NTC has been unable to form a caretaker government because of wrangling over portfolios.

It could now miss the latest deadline it gave itself for naming a government, within the next few days, a source close to the council told Reuters.

Rite Aid 2Q loss narrows, 2012 forecast improves (AP)

Rite Aid Corp. said Thursday its fiscal second-quarter loss narrowed as a customer rewards program improved sales and Hurricane Irene brought the drugstore operator more business at the end of the quarter.

The Camp Hill, Pa., company also trimmed its projected loss for fiscal 2012.

Rite Aid lost $94.7 million, or 11 cents per share, after paying preferred dividends in the quarter that ended Aug. 27. A year ago it lost $199.3 million, or 23 cents per share, when it absorbed a $44-million charge tied to refinancing. Revenue climbed nearly 2 percent to $6.27 billion.

Analysts surveyed by FactSet expected, on average, a loss of 18 cents per share.

Rite Aid operates about 4,700 stores and trails only CVS Caremark Corp. and Walgreen Co. in size. Unlike those competitors, Rite Aid has a string of quarterly losses dating back to 2007.

The chain has closed hundreds of stores in recent years and refinanced debt to improve its financial position. Sales at its remaining stores are improving.

The company said earlier this month that sales at stores open at least a year rose 2.2 percent in the quarter, as non-pharmacy sales rose 2.5 percent while pharmacy revenue was up 2 percent. This is a key indicator of a retailer's long-term health because it excludes stores that recently opened or closed. Rite Aid broke a long streak of declines in that figure last winter.

CEO John Standley told analysts Thursday morning that the company's Wellness Plus customer rewards program was a key factor behind the growth. The program was launched in April 2010, and Rite Aid said it now has more than 44 million members, up from nearly 40 million reported for the first fiscal quarter.

They accounted for 66 percent of the company's prescriptions and 69 percent of its non-pharmacy sales during the quarter.

Rite Aid also saw 2.5 percent sales growth from stores open at least a year in August, when customers stocked up on non-pharmacy items like food and batteries in preparation for Hurricane Irene. But the company said Thursday it gave back much of that storm-related gain in September, when customers stayed away after the hurricane swept up the East Coast.

Rite Aid also said Thursday it was putting a new Save-A-Lot store concept on hold. The company announced the idea, which adds a grocery section to drugstores, last September and said it would test it at 10 stores in and around Greenville, S.C.

Standley said Thursday the profit margins of those stores have not been strong enough to take advantage of the sales gains they create, and Rite Aid doesn't plan to expand the concept with its partner, the grocery chain Supervalu.

"We still like the grocery concept and think it can work in certain of our stores," he said.

The company also started changing some of its stores to a new wellness format in the fiscal second quarter. These stores have more organic food, natural personal care products, and homeopathic medicines, along with employees who help customers find vitamins and nutritional supplements, and carry tablet computers to look up products for shoppers.

Rite Aid completed 40 wellness stores in the quarter, and Standley said customers have told them they like "the look and feel" of the stores.

For fiscal 2012, Rite Aid now expects a loss of between $345 million and $495 million, or 40 cents to 56 cents per share, and sales that will range between $25.8 billion and $26.1 billion. That compares to its previous forecast for a loss of between $370 million to $560 million, or 42 to 64 cents per share, and sales ranging from $25.7 billion to $26.1 billion.

Analysts forecast an annual loss of 50 cents per share and $25.6 billion in sales, according to FactSet.

Rite Aid shares were down 3 cents at $1.04 in afternoon trading while the Dow Jones industrial average slid more than 3 percent. The company's shares have climbed more than 18 percent since closing 2010 at 88 cents.

Lester Holt new host of 'Dateline NBC' (AP)

NEW YORK ? Lester Holt will replace Ann Curry as host of the newsmagazine "Dateline NBC" when it begins its 20th season on Sept. 23.

The NBC News veteran will keep his job as co-anchor of the "Weekend Today" show. Curry, who is now co-anchor with Matt Lauer of the first half of the weekday "Today" show, was judged too busy for both jobs.

The announcement was made Monday.

Holt has been a valued utility player at NBC News, doing multiple reporting and anchoring jobs at the network and sister station MSNBC, and he said people often come up to him wondering when he takes time off.

"Sometimes, I'll be flipping the channel at home and think, `Wow, there's a lot of me on TV,'" said Holt, who said his weekends are busy but he has a lot of schedule flexibility otherwise.

He said he'll report stories for "Dateline" as well as anchor, and his hour-long show about the economy was well-received this summer. He's used to reporting quick pieces for NBC's "Nightly News" and "Today," and the longer-form journalism will give him the chance to do something new, he said.

"He's a terrific anchor, he's an excellent reporter and a great team leader ? all the things you need to do to run a show," said David Corvo, senior executive producer in charge of NBC's newsmagazines.

"Dateline NBC" airs on Fridays in the fall, most often focused on crime mysteries. The menu broadens after pro football season, when "Dateline" rejoins the schedule on Sunday nights, but usually stays as a single-topic hour. In the spring it runs twice a week.

The newsmagazine itself has been a valued utility player at NBC, expanding and contracting to air multiple days a week or just one. There was some question about whether it would make a third year, much less 20th, following the 1992 scandal where the show rigged a fiery truck crash to expose defects, a misdeed that led to the resignation of the NBC News president at the time.

This year it risks being overshadowed at its own shop by the development of "Rock Center," a multi-topic newsmagazine being led by top NBC anchor Brian Williams. "Rock Center" has no scheduled debut or day on the schedule and will replace a failed NBC entertainment program in prime time, meaning its premiere is likely this fall.

Holt said the question of how "Dateline" would co-exist with a new program likely to have considerable promotion time was the first he asked when approached about the anchor job.

He said he was convinced that it would work and that the development of the second show illustrates a commitment to news by NBC's corporate bosses.

"There's a lot more creative energy in the building, which can only be good," said Liz Cole, executive producer of "Dateline." She's pleased that "Dateline" has shown some ratings energy this summer, most recently with Tom Brokaw's special looking back at Sept. 11, 2001.

Holt said, "There's room for both, and there's room for both to grow."

PETA to launch porn site in name of animal rights (Reuters)

NEW YORK (Reuters) ? People for the Ethical Treatment of Animals, no stranger to attention-grabbing campaigns featuring nude women, plans to launch a pornography website in the name of animal rights.

The nonprofit organization, whose controversial campaigns draw criticism from women's rights groups, said it hopes to raise awareness of veganism through a mix of pornography and graphic footage of animal suffering.

"We're hoping to reach a whole new audience of people, some of whom will be shocked by graphic images that maybe they didn't anticipate seeing when they went to the PETA triple-X site," said Lindsay Rajt, PETA's associate director of campaigns.

PETA has been accused of campaigning for animal rights at the cost of exploiting women. A Facebook group, Real Women Against PETA, was launched after the organization paid for a billboard showing an obese woman with the message: "Save the Whales. Lose the Blubber. Go Vegetarian."

Another critical Facebook group is called, "Vegans (and Vegetarians) Against PETA."

"PETA is extremely disingenuous," said Jennifer Pozner, executive director of the New York-based advocacy group Women In Media & News. "They have consistently used active sexism as their marketing strategy to garner attention. Their use of sexism has gotten more extreme and more degrading.

"This may be in their minds the only thing left at their disposal to lower the bar," she said.

PETA has filed paperwork to launch its pornography site when the controversial new .xxx domain becomes active in early December. While many nonprofits and corporations are scrambling to protect their website names from being hijacked by a pornographer slapping on a .xxx domain, PETA is embracing the new domain as just another way to conduct business.

"We try to use every outlet that we can to speak up for animals," Rajt said. "We anticipated that this new triple-X domain name would be a hot topic and we immediately decided to use it and take advantage of it to try to promote the animal rights message."

Jill Dolan, director of the program in gender and sexuality studies at Princeton University, was critical of the PETA campaigns.

"Exploiting porn to get people's juices going seems lame; exploiting pornographic images only of women to make their point is retrograde and misogynist," Dolan said in an email. "Come on, PETA. Don't be Neanderthals."

Rajt denied that PETA has been insensitive to women.

"Our demonstrators, the models, all chose to participate in our campaigns... It's not a very feminist thing to do to turn to women and tell them whether or not they can use their voices, their bodies to express their voice."

Visitors to the X-rated site will initially be presented with pornographic content as well as images from PETA's salacious ads and campaigns, Rajt said. Those images will be followed by pictures and video shot undercover of the mistreatment of animals. The site will also include links to vegetarian and vegan -- using no animal products -- starter kits as well as recipes.

PETA's ad campaigns have featured adult film stars Sasha Grey, Ron Jeremy and Jenna Jameson. In 2008, the organization's YouTube account was temporarily shut down after showing racy videos of celebrities and others posing nude.

"When people first visit the site, it will be very enticing and once they go just a little bit deeper, that's when they'll be confronted with images that we hope will make them stop and think and get them talking and hopefully encourage them to make a lifestyle change to a plant-based diet," Rajt said.

I was watching a commercial this morning that brought tears to my eyes and has brought back a lot of hurtful memories from when I was in school and even now days and what school is like. It's bullying, teasing, calling names, making fun, whatever and however you choose to look at it. Kids these days are just mean and evil at times and it's severely damaging to others or at least those it's done to.
Here's the deal. As a child and teen I didn't have the "cool" clothes or shoes or whatever else was "in" as they call it. I was just me. I came from a family of 4 kids and didn't have a lot of money but what I had to me was awesome and I guess that wasn't enough for those"cool" kids. I was made fun of for wearing "out of style hand-me-downs" but I think Dolly Parton said it best when she sang "Coat of Many Colors" everything my family gave me was worn proudly. Well I am 36 years old and things are exactly the same way. NOTHING has changed, there are still all these little clicks and bff pacts and what not that happen and unfortunately there isn't a whole lot we can do to stop it but I for one have always tried to teach both my girls RESPECT, KINDNESS, and to NEVER pick on or poke fun at others. The band Alabama released a song several years ago called Angles Among Us and if you listen to the song it talks about how there could at any point be angles around us. Well that got me to thinking you never know, that little girl you decide to pick on could be an angle God sent to test you. Will you pass the test? Words are a very hurtful and powerful thing, you don't know people life and or background. You don't know what happens to them at home and you also don't have the right to hurt people in any way. Freedom of speech doesn't give you a free pass to be mean but if that's the choice you make then it's you I feel sorry for. I had an amazing teacher in high school teach me a very valuable lesson and from that day on I knew I would never do to people what they did to me. The golden rule " Do unto others as you would have them do unto you" words to live by. So ya see I wasn't the cutest but I was as far as I was concerned. But I don't think I turned out to bad.
